CPA Forest Park Golf Scramble Registration/Sponsorship Form

Our annual golf scramble tournament will be hosted on 7/27/23 at Forest Park Golf Course loacted at 6141 Lagoon Dr, St. Louis, MO 63112. Lunch and check in will start at 11am with a 12pm shot gun T time. Lunch, beer, water and soda are included through out the event. Parking is on the main lot of the course and the lunch pavillion is adjacent to the lot.
